The questionnaire is provided by the Occupational Therapist to the child’s parent(s) or guardian(s) to obtain their opinion about how the child performs in major assessment areas such as attention and behaviour, play and social skills, self-care, hand/fine motor skills, body and gross motor skills and sensory skills.
The parent(s) or guardian(s) identify areas of strength and areas for improvement observed in the home environment. This information is considered as part of the overall assessment of the child including feedback from the preschool staff and other healthcare providers to develop therapy goals and priorities.
